中译英 (1)建立了国内体外膜式氧合技术(ECMO)治疗重症呼吸衰竭的技术体系。牵头进行了国内第一项ECMO多中心临床协作研究,成立了国内最大的呼吸衰竭ECMO治疗中心,完成了国内最多例数的ECMO治疗呼吸衰竭的临床工作,目前病例数近120例,并撰写了国内第一部ECMO治疗重症呼吸衰竭的临床应用行业规范《体外膜氧合治疗成人重症呼吸衰竭临床操作推荐意见》。 (2)深入地研究了呼吸机相关肺损伤的发生机制以及肺组织损伤后修复机制,丰富了ARDS的肺保护通气的理论依据,提高了机械通气临床应用的安全性,并撰写了我国第一部《急性呼吸窘迫综合征机械通气指南(试行)》。 (3)提出了慢阻肺急性加重合并侵袭性肺曲霉菌病的早期诊断策略和诊断标准。前瞻性总结了该疾病的临床特点,建立了国内最大的临床与标本数据库,其中2篇发表研究被引为美国感染病学会(IDSA)2016年曲霉菌病诊治指南的参考文献。 (4)建立了多个病种的临床资料与生物样本库,涉及病种包括ARDS(约460例)、ECMO患者(约120例)、慢阻肺合并肺曲霉菌感染(约100例)和卡氏肺孢子菌肺炎(约96例)等,并完善了标本库信息管理系统,为此后临床研究提供了重要的研究基础。 (5)结合上述研究成果,创办了系列呼吸支持技术培训课程,包括气道管理与肺康复、无创通气、有创通气、呼吸力学和ECMO等,每年6-8期,培养学员1200余人/年,成为国内重要的呼吸支持技术培训中心,有力促进了我国呼吸支持技术临床应用水平的整体提高。 (1) Establishing the extracorporeal membrane oxygenation technique (ECMO) system for severe respiratory failure in China. We conducted the first multi-center clinical collaboration study of ECMO in China, set up the largest ECMO treatment center for respiratory failure in China, and accomplished the nationally greatest number of ECMO for the treatment of respiratory failure, with nearly 120 cases at present. Moreover, the nationally first guidelines about clinical application of ECMO in treating severe respiratory failure, Suggestions about clinical procedures of extracorporeal membrane oxygenation in severe adult respiratory failure, was drafted. (2) In-depth study on the pathogenic mechanism of ventilator-related lung injury and repair mechanism following lung injury, providing abundant theory basis for pulmonary protective ventilation of ARDS, and improving the safety about clinical application of mechanical ventilation. Moreover, a book called Guidelines (trial) of mechanical ventilation for acute respiratory distress syndrome was drafted. (3) Proposing early diagnosis strategies and diagnostic standard of acute aggravation of COPD combined with invasive pulmonary aspergillosis. Clinical features of the disease were prospectively summarized, and the nationally largest clinical and specimen bank was established. 2 papers were cited as referencefor guidelines of aspergillosis diagnosis and treatment issued by Infection Disease Society of America in 2016. (4) Setting up clinical data and biological specimen bank of various diseases, including ARDS (about 460 cases), ECMO cases (about 120 cases), COPD combined with pulmonary aspergillosis (about 100 cases) and pneumocystis carinii (about 96 cases), etc. The information management system of the specimen bank was completed, providing an important basis for the future clinical study. (5) Based on the research achievements above, a series of respiration supporting technique training courses, including airway management and lung rehabilitation, noninvasive ventilation, invasive ventilation, breathing mechanics, and ECMO, etc., which were provided for 6-8 times each year, were created. Over 1,200 students were cultivated each year, and this center became an important respiration supporting technique training center nationally, promoting clinical application of respiration supporting techniques in China. |
